The Site supports our GLP-1 weight management program and the in-studio services that come with it. To do that, we collect several categories of information from and about the people who use it, including the details you share during intake, your contact information and your general location.

• Determine whether our program may be a fit for you and route you to a licensed provider for review.
• Coordinate fulfillment and shipping of medication prescribed by a licensed provider.
• Connect you with your local Restore studio for included services and RN support.
• Send program updates, appointment reminders and account notices.

Depending on how you use the Site, we may collect the following categories of information directly from you:

• Identifiers such as your name, email address, phone number and mailing address.
• Health-related information you provide during intake, including height, weight, medical history and current medications.
• Program and transaction information, including the stack you select, billing records and shipment history.
• Location information such as your ZIP code, city and state, used to match you with a nearby studio. We do not access precise device geolocation unless you grant permission.
• Communication preferences and the content of messages you send us.
• Technical data about your device, browser, operating system and how you interact with the Site.

We also collect information automatically through cookies and similar technologies, and we may receive information about you from pharmacies, providers, payment processors and service providers who help us operate the program.

We use the information we collect for purposes including:

• Operating the Site and providing the features and Services you request.
• Facilitating clinical review by board-certified providers and coordinating prescriptions with participating pharmacies.
• Scheduling and delivering the in-studio services included with your stack.
• Processing payments, preventing fraud and maintaining account security.
• Responding to your questions and providing customer and RN support.
• Improving the Site, measuring performance and understanding how it is used.
• Complying with legal obligations and enforcing our Terms of Use.

Where permitted by applicable law, we may also use your information to send marketing communications about our programs and services, and to personalize the content and offers you see.

We do not sell your health information. Your information may be disclosed to:

• Licensed providers and participating pharmacies involved in your care and fulfillment.
• Restore studios and clinical staff supporting your program locally.
• Service providers who host our systems, process payments, ship orders, deliver messages or provide analytics on our behalf.
• Professional advisors, auditors and insurers.
• A successor entity in connection with a merger, acquisition, financing or sale of assets.
• Law enforcement, regulators or other parties where we believe disclosure is required by law or necessary to protect rights, safety or property.

We may also share aggregated or de-identified information that cannot reasonably be used to identify you.

Information you share with a licensed provider in the course of clinical care is handled under applicable health privacy laws and the provider's own notice of privacy practices. Text messaging consent is never shared with third parties for their own marketing purposes. We require our service providers to use your information only to perform services for us and to protect it with appropriate safeguards.

We maintain administrative, technical and physical safeguards designed to protect the information we collect against loss, misuse and unauthorized access, disclosure, alteration and destruction. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. Please protect your account credentials and notify us promptly if you believe your account has been compromised.

You can:

• Unsubscribe from marketing emails using the link in any message, or by emailing us.
• Reply STOP to any text message to stop program-related SMS.
• Ask us to review, correct or delete information we hold about you, subject to legal and clinical recordkeeping requirements.
• Adjust your browser or device settings to limit cookies and tracking.

You may continue to receive non-promotional messages about your account, orders or safety notices even after opting out of marketing.

The Site and Services are intended for adults 18 years of age and older. We do not knowingly collect information from children. If you believe a minor has provided us information, contact us and we will take reasonable steps to delete it.

California residents have additional rights regarding personal information, including the right to know what personal information we collect, use and disclose, the right to request deletion or correction, and the right to opt out of certain sharing.
